Customer information, declaration, and consent forms
Prior to your pet’s stay at Cow Hill Kennels, you will be required to complete our customer information form, with your full contact details, and information relating to each pet.
In addition, you will be required to complete our declaration and consent form.

There are several ways you can complete the form:
1. Complete the form online, download ‘with changes’, save the document to your desktop and return it to us via email.
2. Print out a copy of the form, complete and sign and return via post or by hand.
2. Complete at Reception.
